When you become a new patient at Karam Family Dental, there is some information we need from you so we can give you the best possible dental experience.

Please download and complete the Patient Registration, Medical History, and HIPPA forms and bring them with you on your first scheduled appointment. Or, if you prefer, you may fill out these forms in our office when you come for your first scheduled appointment.
